Endependence Center - Survey

 
  The ECI Consumer Advisory Committee (CAC) is comprised of people with disabilities that
  are receiving Center services.  Annually the CAC develops a Participant Satisfaction Survey
  form and it is distributed to active participants of the Center.  The purpose of this survey is
  to gather information and feedback about your experience at the Endependence Center. 
 
  Thank you for completing this survey!
 
  The information you provide will be helpful to ECI in both planning needed services and
  assuring the quality of current services.  The CAC Committee members compile the results
  of the survey and submit a written report to the Center’s Executive Director, management
  staff and Board of Directors for review and to aid in planning.
